The following checklist is vital for whatever workplace that has lift trucks on the premises. Each of these questions should be answered with "yes" if all the safety regulations and rules are being followed. A "no" to whatever question indicates a procedural weakness that could result in accident, injury or damage to machinery.

1) Is the pedestrian traffic separated from the forklift working area?
2) Is access to forklift areas by pedestrians restricted by a barrier?
3) The pedestrian walkways well-marked?
4) Are "forklift in operation" signs visibly clear?
5) Are mirrors for drivers and pedestrians provided at blind spots?
6) Are changes to daily forklift schedules communicated to all affected workers?
7) Are forklift tires suitable for utilization on all operating surfaces?
8) Is the seatbelt being utilized?
9) The forklift seat is in good condition?
10) Is a regular forklift maintenance schedule adhered to?
11) Is a yearly forklift inspection and certification done?
12) Is the forklift right for the required job?
13) Is there a readable load chart on the forklift?
14) Do all operators understand how to read the load chart?
15) The chart lists the safe lifting capacity for all attachments and their uses?
16) Are the warning devices on the lift truck working correctly?
17) The speed operating control device is secured correctly?
18) Is overhead cage fitted correctly on the lift truck?
19) Do the operators of the forklift have certifications to use the machinery?
20) Warning signs and speed limits are being observed?
21) The operators have current training for the lift trucks being utilized?
22) Forklift operators know their duties to report the damage to the machine and other forklift accidents?
23) Prior to the machinery being used, the pre-test checklist is being reviewed on a daily basis?
